Mikoto Uchiha is the wife of Fugaku Uchiha, who was the head of the Uchiha Clan. She settled down and started parenting their two boys, Itachi and Sasuke, as a housewife. She often met up with Kushina Uzumaki while out and about and introduced Sasuke to her.
When Biwako pointed out that they had named him after the Third Hokage's father, Mikoto said that they had done so in hopes that he would become a shinobi as powerful as his namesake.
When Kushina inquired as to whether or not delivery was painful, Mikoto chuckled, having discovered the one thing that Kushina seemed to be terrified of. She concurred with Kushina's expectations that their boys would become friends. She is one of the few who knew that Naruto is the son of Minato and Kushina.
Itachi saw that she and Fugaku weren't at home before the Nine-Tailed Demon Fox's attack. Itachi and Sasuke were uninjured when Mikoto came home following the attack in Itachi Shinden: Book of Bright Light.
The Nine-Tails' assault debris was hurled their way as soon as they arrived. Itachi shocked her with his display of power as he instantly destroyed the debris to save Sasuke and Mikoto. She was deeply devastated by the loss of her good friend and also visited Kushina's newborn baby Naruto Uzumaki at the hospital after the Nine-Tails was halted in the anime.
Itachi had left her and Fugaku till the end and approached them as the clan was being destroyed. She did not attempt to protest as she knelt down next to her husband.
She just said that she understood when why he tried to defend himself before letting an upset Itachi murder them. This would eventually drive Sasuke to desire exacting revenge for her murder and the deaths of the other clan members.
